History and Physical Forms
Mobile providers have the ability to fill out the H&P via a handheld device. Once completed, providers fax or email the form to their patients’ facility or insurance company. The providers’ billing or administrative personnel has instant access to the MediMobile™ Forms via MediMobile™ Web Solution to begin the billing process. The MediMobile™Form is associated to the patient record. On the next visit, the provider will begin with a fully populated form from the last visit, change appropriate information, and repeat the process.

Provider A refers a patient to Provider B. Provider B is a MediMobile™ user and performs service on the referred patient. When Provider B completes visit, Provider B creates the bill. Once the bill is completed, Provider B tells the MediMobile™ system to send the Discharge letter. The MediMobile™ Form auto populates the referring doctor’s name, patient name, location, all procedures and diagnosis performed. Provider B has the option to enter specific notes into system. Once completed, Provider B either faxes or emails the letter to the referring physician. The completed process takes less than 10 seconds.
